Mocha recipe?

Hello I used to work for Arabica in Cleveland and I am trying remember what the recipes is for their Mocha, I know that Phoenix Coffee uses the same recipe. I know the ingredients (half & half, Hersey syrup, milk and toddy) but I need the correct measurements needed. Can someone help me out?
